Directions
for Completing Template Request Forms
At this time, there
are three ways for you to choose to submit your template
request.
First you may copy and
paste this form into your e-mail and send it to Patience
Terrizzi (
Patience Terrizzi ) at the CLAN office.
Second you may download
this form into Word and then e-mail it as a an attachment to the
above e-mail.
Finally, you may photo copy
it and send it via courier to the CLAN office.
Remember we work on these as a first come first served basis so
e-mail will be received prior to requests that come in the
delivery.
First complete the date, library, contact name and phone
number. This way we can prioritize our requests and contact the
correct person if we have additional questions.
Next create a title and description for your template. The
title should be an abbreviated form of the description that will
be different from other libraries. For example if you are in
Warwick and want a patron template for children your title may
be warjuv, for Warwick juvenile. In the description, you can
indicate the meaning of your title.
Choose the fixed-field and variable-field elements you wish to
include in your template. *
Next, you may choose to have each field left blank for your
staff to fill in or you can opt to pre-populate the field with a
default that can be typed over. For example in the
bibliographic template you may want the language to always come
up as eng (the Library of Congress code for English). If anyone
wants to change the code they would have to type over the
existing code.
Finally you can choose to automatically prompt for any of these
fields. In other words, as the person inputs information into
the template, the fields that you have selected to be prompted
will pop up in a box and drive the person to either input
information or choose not to. Automatically prompted fields
should be the ones that are required to make the template
legitimate for your purposes.
There is a space provided on the bottom for you to enter any
additional information pertaining to your template request.
